POPE JOHN PAUL II CATHOLIC SCHOOL ATHLETIC POLICY HANDBOOK FOR ATHLETES AND PARENTS

All students in grades 6-8 in good academic standing are eligible to participate in after school sports programs. Fifth graders may play golf, volleyball, cheerleading, and basketball. They are invited to participate in soccer and tennis if additional athletes are needed to field teams.

REQUIREMENTS FOR PARTICIPATION

Parent attendance at beginning of year Athletic meeting (see dates below)
Parent-signed Athletic Events Consent and Release Form
Completed Medical History Form
Current Sports Physical (see date below)
A nominal Sports Fee is payable per sport, scholarships available

There is MANDATORY meeting for all families whose students wish to participate in Intramural sports for the 2012-2013 school year.

When: Wednesday, August 22, 2012
(you may attend either meeting but you MUST attend one of them if your child wishes to
participate in sports)
Time: 6:00pm in cafeteria
What: Meeting for Sports Parent and Student Athletes to go over the Athletic_Handbook and Forms that must be signed BEFORE a student may participate in a sport.

Sports Physicals

When: August 24, 2012
Time: 3:15 - 4:30
What Sports Physicals for a donation of $5.00. Students must have signed permissions for physical and sports physical history form in order to receive a sports physical at the school. Forms will be handed out at the MANDATORY meeting (see details above). Students will be seen by physicians in the order the forms are turned in to Coach.
